REFUNDS

- If your child quits BEFORE August 1st, and all equipment is turned in as it was issued, you will receive a full refund.

- If your child quits ON or AFTER August 1st, NO refund will be given and your child will not receive credit for that year of play. (By-Laws X:Par5)


Print the blank REGISTRATION FORM and CODE OF CONDUCT (BOTH PAGES), then fill out and bring the form with you to the sign-ups. Also, you must bring a copy of your child's birth certificate and recent photo.


Registration Forms and Code of Conduct MUST be
completed and SIGNED prior to participant's first practice!


All Fees (Current & past due)
MUST be paid in full at
The Time of Registration!
NO EXCEPTIONS

This year we will be requiring that for ALL Bantam players/cheerleaders, a Parent/Guardian MUST be present at every practice or game.


2011 FEES:
1st Child: $55.00
2nd Child: $45.00
3rd & each additional child: $35.00

Children must be from the
same immediate family.

All football players must have their birth certificate copy and picture no later than  the first practice

VARSITY
Ages 11-12;
birthday must be between
Sept 01, 1997 to Aug 31, 1999



JUNIOR-VARSITY
Ages 9-10;
birthday must be between
Sept 01, 1999 to Aug 31, 2001



BANTAM
Ages 6-8;
birthday must be between
Sept 01, 2001 to Aug 31, 2004
